Q: What is Acid Dump?
A: Acid dump is the release of stomach acid in relation to a stimulus. For example, It occurs when sodium bicarbonate is used as a buffer with too little PPI. It can also occur when an antacid is used alone. There is also a nocturnal dump of acid, called nocturnal acid breakthrough (NAB). There is a post-feeding dump of acid known as post postprandial acid reflux. Basically, it is "production of acid" in a situation where there is little to nothing to block the acid.
Q: What is Carafate
A: Carafate is sulfated sucrose with aluminum hydroxide. It requires acid to work (bad); it requires 4 doses per day or more. It can only stick to an open ulcer (won’t stick to just slightly eroded mucosa). It has no ability to block the production of acid.
Q: Can someone explain to me how reflux and red face go together? My 4 month old seems to get red in the face at the end of the day. She has reflux and laryngomalacia.
A: Histamine causes flushing of mucosal membranes and in the face, due to the high vascularization of the face. Histamine, by it's very nature causes vasodilation and recruitment of eosinophils. Acid reflux into the oropharynx and airways is a powerful stimulant of histamine release.

A: 5 or 6 times
Q: What is FPIES:
A: It is a non ieg allergy and the trigger can be any food. That is the scary thing, we don't know what they will react to until they do. There are acute reactions that follow a "typical" pattern of vomiting 2-5 hours after ingestion but the vomiting doesn't stop...ever. They vomit to bile and at that point they are dehydrated and their heart rate increases or BP decreases. Some kiddos will run a fever from going into shock. My girls will become so lethargic they don't respond to needles, IV etc. There are also chronic reactions which can include some vomiting but also rashes, bloody stools etc. We had GI bleeds and severe vomitting from peas and pumpkin. And shock with dairy, soy and sweet potatoes. They haven't tried seafood or pork. My youngest is less severe.
Q: What is Bethanechol?
A: Bethanechol is a cholinergic drug that is supposed to enhance the sphincter muscle between the stomach and the esophagus and enhance the motility of the Gastrointestinal system. It is not effective and causes cramping
Q: What is non-acid reflux?
A: It is a misnomer, it is really acid reflux between pH 4 to 6. Traditional "acid reflux" is pH <4, so this term came about "non-acid reflux" if the pH was higher than that.
